Counseling Relationship. Our purpose is to examine and change unhealthy behaviors.
Qualifications. I am a Licensed Professional Counselor (LPC license #1124), a Licensed Marriage and Family Therapist (LMFT license #128) and licensed and ordained Clinical Pastoral Counselor. I hold the Bachelor of Arts (B.A.) degree from Samford University, the Master of Divinity (MDiv.) degree from The Southern Baptist Theological Seminary, and the Doctor of Ministry (DMin.) degree from Southeastern Baptist Theological Seminary. I’ve pursued further studies at Regents Park Baptist College at Oxford University (England), Ruschlikon Seminary (Switzerland), Louisiana State University in Baton Rouge, The University of New Orleans, New Orleans Baptist Theological Seminary, and The University of Massachusetts.
Work Experience. For over 30 years, I’ve maintained a private practice in individual, couple and family counseling. Throughout these three decades, I’ve also served several faith communities as a pastor and denominational leader, but now choose to focus my life’s work entirely in the field of Behavioral Health. For over 16 years, I served on the clinical staff of a private psychiatric hospital and, since 2011, my weekday responsibility has been as Director of Behavioral Health for a Primary Healthcare Network (www.excelth.com) with three integrative health centers in New Orleans, LA, and a free-standing Behavioral Health center in Baton Rouge.
Counseling Techniques. Cognitive Behavioral Therapy, Family Systems Therapy, Short-Term Solution Focused Therapy, Motivational Interviewing, Behavioral Activation Therapy, Pastoral Counseling and Neurolinguistic Programming.
Professional Fee. $135 for initial consultation and $120 per hour subsequently, unless contracted otherwise.
Cancellation Policy. If you must cancel or reschedule an appointment, please do so no later than 24 hours in advance, by texting or calling (985) 778-6049, or emailing me at firstname.lastname@example.org.
Insurance. I take Blue Cross, Gilsbar 360 and Beacon insurances. To verify insurance coverage call my billing service at (504) 309-7844 prior to scheduling your appointment. Please be aware there is no guarantee of payment by an insurance company and you are responsible for the professional fee.
Types Of Services Offered and Clients Served. My counseling practice is with adults, adolescents and older children, in individual, marital or family therapy sessions, for the treatment of depression, anxiety, panic, phobias, complicated bereavement, painful life-transitions, post-traumatic stress, marital and family conflicts, career decisions, low self-image, habit-control and a wide range of other common human problems.
Code of Conduct. My services are provided in accordance with The Code of Conduct for Licensed Professional Counselors and Licensed Marriage and Family Therapists, as set forth by the Louisiana Licensed Professional Counselors Board of Examiners. A copy of either ethical code is available upon request. The address of the Louisiana Licensed Professional Counselors Board of Examiners is 8631 Summa Avenue, Suite A, Baton Rouge, Louisiana 70809, and the phone number is (225) 765-2515. Their FAX number is (225) 765-2514. Here is an electronic link to the LPC Board: www.lpcboard.org
Privileged Communication. When working with a family, information shared by individuals in sessions where other family members are not present must be held in confidence from third parties and not revealed to any third party unless all individuals involved sign written waivers at the outset of therapy.
Couples and families are treated as one unit. If there is something you definitely do not want me to share with another member of this unit in therapy, it is your responsibility to let me know that clearly.
Emergency Situations. If you are having a psychiatric emergency, especially with thoughts of self-harm or harm to someone else, please go to the nearest emergency facility or call 911. The National Suicide Prevention Hotline is 1 (800) 273-TALK (8255). I cannot promise my immediate availabilty to talk with you, although I do want to assist in supporting you once the immediate danger has passed.
Your Responsibilities. If you have been in counseling with another mental health professional, the Code of Conduct requires me to ask that you: (1) Inform me of your other ongoing professional mental health counseling relationship; (2) Consider formally terminating the counseling relationship with the other therapist before beginning counseling with me; or (3) Allow me to communicate with your other mental health counselor (psychologist, social worker, etc.), if it seems in your best interest.
Physical Health. If you have not had a physical examination in the past year, it is strongly recommended.
Potential Counseling Risks. As a result of mental heath counseling, you might realize you have additional issues which have not surfaced prior to the onset of the counseling relationship.
